EvoLab Physical Therapy · Good Faith Estimate
Good Faith Estimate.
What the No Surprises Act entitles you to, and how we handle it here.
Under the federal No Surprises Act, if you don’t have insurance, or you’re not planning to use insurance to pay for your care, you have the right to receive a Good Faith Estimate of what your care is expected to cost, before you receive it.
What a Good Faith Estimate is.
It’s a written estimate of the expected charges for the services we reasonably expect to provide you, based on the information available at the time you schedule or request it. It is an estimate, not a bill or a final price, since your actual plan of care is set at your evaluation and can change based on what we find.
How we handle it.
If you’re uninsured or self-pay, we’ll provide your Good Faith Estimate in writing before your first scheduled visit, and again any time your care plan changes materially. It will include the expected cost of the evaluation and the likely course of treatment, itemized by service.
Your right to dispute.
If your final bill is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill. You have 120 days from the date on the original bill to start a dispute. There is a $25 fee to use the dispute process. If the agency reviewing your dispute agrees with you, you will have to pay the price on your Good Faith Estimate. If the agency disagrees with you and agrees with the health care provider, you will have to pay the higher amount.
Keep your copy.
Keep a copy of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, or how to dispute a bill, visit www.cms.gov/nosurprises or call 1-800-985-3059.
Request yours.
Call (910) 218-8108 or email nate@evolabpt.com to request a Good Faith Estimate before you schedule, and we’ll have one to you before your visit.
Put it in writing.
Five short questions. A real person calls you back, usually within two business hours, with times that fit your week.
The people who end up filling this out are rarely in the worst pain of their life. They’re just done working around it, done picking the errand that doesn’t require the stairs. If that’s the stage you’re at, this is the part where you stop researching and start.
Consider it filed.
